Latest Newburgh Water Test
Here is the report I just received from Ward Lab. This is a water sample straight out of the tap.
Calcium, Ca 81
Magnesium, Mg 21
Bicarbonate, HCO3 209
Sodium, Na 26
Sulfate, SO4-S 39

This is what I'm currently using: http://www.ezwatercalculator.com/
I usually add 5 gallons of walmart RO water and the difference is charcoal filtered tap water. I shoot for like 60-90% RO water. Having some tap water adds in some trace stuff that get stripped out of 100% RO. The spreadsheet will show you what the resulting salt concentrations and ratio will be after the dilution of soft RO water.
I completely ignore Residual Alkalinity (RA) and only adjust for the Chloride/Sulfate ratio. It's like adding seasoning to cooking for me. RA is too much smoke and mirrors for me but a lot of people rely on the RA number, including Palmer.
I only adjust with Gypsum (CaSO4) and Calcium Chloride (CaCl2) to get the Chloride/Sulfate ratio to what is appropriate for the style I'm brewing but keeping their total concentrations relatively low. Adding the RO water keeps them low since you are starting with less in the water.
I adjust the pH with a little Acidulated Malt in my mash. This is also in the spreadsheet.
Example: Hoppy wheat, 5 gallon BIAB
Total water: 8.33 gal
RO: 5 gal (dilution 60%)
Tap: 3.33 gal
Added 2g CaSO4 + 3g CaCl2 for a ratio of .72 (which is skewed to the hoppy Sulfate side)
Total salts were 52 ppm Chloride and 73 ppm Sulfate.
Added 5oz (3.1% of total mash) of Acidulated Malt to bring mash down to 5.51 at room temp.

I thought your Sulfate was really low until I remembered that Ward reports it as SO4-S. Multiply by 3 to get SO4. =117.

At 100% tap water, 33 ppm of Chloride to 117 ppm of Sulfate is a ratio of .28 which I would consider way out of balance even for a really hoppy beer. At minimum you could just add in like 5 or 6 grams of Calcium Chloride CaCL2 for a 5 gallon batch and have reasonable balanced water. Add less for a hoppy beer and more for a malty one.

Does this have to be requested through a lab? Or can I call Indiana American Water and request the information?

Ok, so I emailed Indiana American Water and requested test results for the following (which I got from Ward Labs website)
Sodium, Calcium, Magnesium, Potassium, Carbonate, Bicarbonate, Chloride, Iron, Sulfate, Nitrate, Electrical Conductivity, Est.Total Dissolved Solids, pH, Total Hardness (Lime), Total Alkalinity & Total Phosphorus
