Some people said this wasn't working, I just tried this link and it still worked for me.
I have a Employee Permit, I would like to hear from someone that has a Volunteer Permit.

You have to put in your last 4 SS# and mm/dd/yyyy
I think it then check its records before it continues.

Volunteer permit issued just as easy. Is the club collecting the Employer copy?

From what I understand the club does not need the employer copy. You just need to have your copy on your person when you are serving. This goes for your licenses and your training certificate.
